二、主要学术成就<= /span>

申请人现任浙医二院肝胆胰外科副主任,主任医师、副教授,主要从事肝胆胰外科和肝脏移植,留学西澳大学、美国约翰霍普金斯医院等。主持科研项目12项,其中国家自然科学基金4项;发表学术论文70篇,其中SCI论文50篇,第一或通讯作者SCI论文24篇,影响因子大于5分12篇。获浙江省高层次创新人才,教育部高校科学技术进步奖一等奖1项(排名第二),浙江省医药卫生科技进步一等奖(排名第二);国家专利2项(排名第三)。

申请人长期从事肝胆胰外科的临床应用及基础研究,近年来,在包括4项国家自然科学基金等十余项研究基金的资助下,主要围绕肝胆胰肿瘤进展机制及其治疗策略、肝胆胰外科加速康复等方面开展了一系列创新性的研究工作,近5年发表SCI论文49篇,其中(共同)第一/通讯作者发表SCI论文27篇,影响因子大于5分的有10篇,分别发表在《Cancer Research》、《Oncogene》、《Carcinogenesis》等肿瘤研究领域的TOP期刊,被包括Nature Reviews Drug Discovery(2016,IF 47)和Nature Reviews Immunology (2015, IF 39)在内的SCI收录期刊他引336次。获浙江省高层次创新人才,教育部高校科学技术进步奖一等奖1项(排名第二),浙江省医药卫生科技进步一等奖(排名第二);国 家专利2项(排名第三)。主要学术成绩、创新点及其科学意义如下:

(一)从肿瘤微环境包括低氧、血管、免疫以及肿瘤细胞本身等入手,系统地研究了肝癌、胰腺癌进展机制,为发展有效的干预策略奠定了强有力的理论基础。

(1)从低氧微环境角度,研究肝癌细胞在低氧微环境下发生侵袭和转移的分子机制,证实了在低氧状态下肝癌细胞发生上皮间质转化(EMT),并上首次发现低氧诱导因子(HIF)-1α与β-catenin相互作用在肝癌EMT和侵袭、转移中的作用,提出了低氧-β-catenin-Hif-1α-EMT调节网络(Carcinogenesis, 2013, 共同第一作者,IF 4.874, SCI他引41次);Nature Reviews Drug Discovery(2016,IF 47.12) 述评文章认为本研究对以EMT为靶点进行药物开发具有提示意义。发现低氧状态下发生EMT的肝癌细胞分泌出CCL20,诱导IDO在肿瘤相关巨噬细胞中上调,并且抑制T细胞的功能和增殖(Cancer Research,2016,共同第一作者,IF 8.556, SCI他引2次),首次阐述了肝癌细胞与微环境的交互作用,进而促进免疫逃逸发生转移,这些发现阐释了低氧微环境中肝癌恶性程度增加的新机制,解释了诱发缺氧等临床治疗手段失效的原因,为后续治疗打下基础。

(2)从血管微环境角度,发现肌细胞增强因子2C(MEF2C)在肝癌血管新生中发挥复杂调控作用,证实MEF2C参与VEGF介导的血管生成和肿瘤侵袭,表现出促癌的作用;亦发现MEF2C通过直接与β-catenin相互作用,表现出对抑癌的作用。MEF2C对肝癌的"双刃剑"作用特点,为开发以MEF2C为靶点的抗肝癌药物提供了理论基础(Oncogene, 2015, 第一作者,IF 7.932, SCI他引8次)。作为主要研究者发现mTOR通路调控肿瘤相关巨噬细胞形成并介导肝癌血管新生和肝癌生长(Cancer Research,2012, IF 8.556, SCI他引32次),为后续以mTOR通路为靶点干预肿瘤血管新生提供新思路。Linke等人在Nature Reviews Immunology (2015, IF 39.416) 综述中肯定了该研究成果。  

(3)从免疫微环境角度,从T细胞免疫组库入手,首次揭示了T细胞克隆在胰腺癌患者与正常人群中的分布差异。这些新发现在外周血中监测肿瘤免疫状态的新思路提供了理论基础(Scientific Report,2015, 第一作者,IF 5.228)。

(4)从肿瘤细胞本身,证实了乙肝病毒感染促进胰腺癌细胞恶性行为增加,阐释了其内在机制即乙肝病毒X蛋白通过调控PI3K/AKT发挥促胰腺癌细胞效应(Cancer Letter,2016,共同第一作者,IF 5.992)。该研究提示对于乙肝感染的胰腺癌患者需要和乙肝感染的肝癌患者同样重视抗乙肝病毒的治疗。

(二)解决肝癌、胰腺癌对现有化疗和靶向治疗的耐药性,探索提高化疗和靶向治疗敏感性的新策略,为临床转化奠定了坚实的基础。

肿瘤血管微环境异常是治疗耐药的重要机制,发现蛋白磷酸酶2A(PP2A)可重塑肿瘤血管微环境,利用PP2A抑制剂LB-100有助于多柔比星等多种化疗药物能够顺利通过重塑的肿瘤血管到达肝癌或胰腺癌内部,增强化疗效果(Mol Cancer Ther, 2014, 第一作者,IF 5.579, SCI他引4次;Cancer Letter 2014, 第一作者,IF 5.992, SCI他引4次)。该研究提出通过联合血管扩张和增加血管通透性的方式提升化疗效果,具有极强的临床转化前景。

低氧微环境造成肝癌对靶向药物索拉菲尼耐药,研究发现PP2A抑制剂LB-100可以增强低氧微环境下索拉菲尼的治疗肝癌的效果,并阐明了其机制即通过活化磷酸化的Smad3,下调Bcl-2导致肝癌细胞凋亡(Tumor Biol, 2015, 共同通讯作者,IF 2.926)。

发现mTOR通路在胰腺癌细胞增殖和化疗耐药中发挥重要作用,首次发现一种新型的mTOR抑制剂OSI-027通过调控mTOR及其下游通路和下调多药耐药蛋白1(MDR1)的表达,改善耐药,增强吉西他滨对胰腺癌的化疗效果(Oncotarget, 2015,共同通讯作者,IF 5.008),该研究揭示了胰腺癌耐药的新机制,为胰腺癌多药联合化疗提供了理论基础。

(三)率先实施肝胆胰外科术后加速康复策略,加速手术患者安全康复,降低术后并发症发生率,缩短术后住院日,并在本院外科系统和全国的肝胆胰外科进行推广。

率先开展了肝胆胰外科术后加速康复(ERAS)临床实践,做为执笔者撰发《肝胆胰外科术后加速康复专家共识(2015版)》(2016),这是中国肝胆胰外科领域第一个ERAS专家共识,同期在《中华消化外科杂志》发“肝胆胰外科术后加速康复实施单中心经验”,显示肝胆胰外科实施ERAS安全可行;针对胰腺手术,使得术后平均住院时间从未实施ERAS时的17.1天下降至11.7天,实施ERAS的依从性越高术后住院日越短,并发症发生率、死亡率、非计划再手术率和再入院率等没有增加(Pancreatology, 2016,第一作者, IF 2.406)。

通过RCT研究,证实胰十二指肠切除术两种最常用的胰肠吻合方式(套入式和胰管对黏膜式)具有相似的总胰瘘发生率,但胰管对黏膜吻合具胰瘘更轻、术后住院日更短(J Am Coll Surg,2016,第一作者, IF4.257, SCI他引1次),为胰肠吻合方式的选择提供了强有力的证据。胆道术后感染发生率高,分析胆汁微生态,发现Oddi括约肌松弛的患者中更易有细菌滋生,和术后感染相关,对有Oddi括约肌松弛迹象的胆道患者术前应注意预防性地采取针对性的抗感染治疗(J Am Coll Surg,2016,第一作者, IF4.257)

实施ERAS加快了床位周转,提高床位利用率,成功经验在我院外科系统进行了推广,并应邀到全国各大肝胆胰中心做经验分享和各种学术会议上做学术报告。也得到了媒体的广泛关注,媒体报道100余篇/次。

2016年成立“中国研究型医院学会加速康复外科专业委员会”,担任副主委和秘书长,“加速康复外科医学”被浙江省卫计委列为医学创新学科,担任后备学科带头人.

三、岗位工作思路&= #21450;预期目标

(一)履行本学科教授岗位职责的工作思路

若能受聘于教授岗位,将认真履行教授职责,紧随国际发展前沿,以打造具有国际影响力的学科为目标,努力做好以下几个方面:讲授本学科核心课程,指导博士生、硕士生,培养出具有较高专业水准和科研素质能力的现代医学人才;正确把握学科发展方向,争取在肝胆胰外科和肝移植方面取得技术创新和术后并发症防治等关键领域的重大突破,开展原创性的临床研究和基础研究,发表具有较高影响力的论文,实现科学研究实力达到国内领先、国际先进水平;根据学科发展需要,组建一支结构合理、创新能力强、掌握国际先进研究手段、具有国际化视野的研究团队,实现团队实力达到国内领先、国际先进水平。    

(二)拟从事的研究方向及其科学意义

结合已有工作基础,受聘后将继续在肝胆胰肿瘤微环境与耐药机制研究、新治疗方法和临床转化研究、个体化精准治疗的标准化及推广等方面开展工作。一方面探究微环境中CAF细胞的角色,通过MEF2C通路干预CAF的形成和功能,从而提高肿瘤微血管密度,改变缺氧微环境,起到抗肿瘤、抗耐药的作用。另一方面,我将继续探究肿瘤微环境中T细胞的调控方式,期待通过特异性干预新型免疫共刺激通路,来实现CD8+T细胞的再激活,达到杀伤肿瘤的目标。最后,外周血中循环肿瘤细胞、DNA和外泌体的检测也是未来的一块研究重点,通过对其捕获扩增、分析研究,开发出 “液体活检”诊断技术,推动肿瘤的早期诊断和个体化治疗。
