二、主要学术成就<= /span>

2.1 <= /span>标志性成果(不&#= 36229;过300字)

申请人专注于基于大数据分析和数学模型预测的传染病防控策略研究。任职以来以第一作者发表领域内高影响力论文3篇(均为学校认定的TOP期刊包括1篇IF>20,1篇IF>10,1篇IF>9)。主要的研究成果包括①构建我国和东南亚地区肝癌发病率预测模型,首次对我国和该地区6个国家的1983-2030的肝癌发病率进行比较分析和预测。②首次构建中国HCV感染疾病负担预测模型,该模型模拟不同治疗场景下中国2004-2050年HCV相关疾病负担的变化情况。③通过统计学模型,系统阐明了全球56个国家儿童肺炎相关死亡率的变化趋势。作为负责人主持国家级项目3项,合同经费近200万,包括国家自然科学基金1项和国家科技重大专项子项目2项。

申请人一直专注于基于大数据分析和数学模型预测的传染病防控策略研究,有扎实的理论技术基础,较好的学术积累。申请人以第一作者在Gastroenterology,Hepatology,Clinical Infectious Diseases等杂志发表SCI论文12篇。主持国家自然科学基金、科技部重大专项子项目等国家级项目3项,参与编制中国人工智能2.0发展战略研究(智能医疗部分)。国际上以大数据为驱动的传染病预警和预测研究发展十分迅速,已经成为一个前沿的研究领域。目前,在我国以肝炎为代表的重大传染病防控形势仍然严峻,对于其流行规模、传播特点以及其未来疾病负担仍缺乏有效的认识。《健康中国2030规划纲要》中明确提出,今后要加强重大传染病防控,特别是要进一步完善传染病监测预警机制,因此申请人瞄准国际前沿同时又立足于我国重大的战略需求,围绕我国的重大传染病防控问题开展基于大数据的预测研究,主要学术成就和贡献如下:

一、构建统计分析和预测模型,首次比较并预测了我国和东南亚7个国家至2030年肝癌的发病率变化趋势,为政府部门卫生资源规划包括预防、诊断和治疗资源的调配提供了重要指导。申请人以第一作者身份将相关结果发表于胃肠肝病学顶级期刊Hepatology(IF=3D14.971)

 全球每年估计有782,500新发肝癌病例和745,500人死于肝癌,其中超过70%确诊的肝癌病例发生在我国和部分东南亚国家,其中绝大部分和HBV感染有关。尽管该地区肝癌发病率极高,但是既往的研究并未对该地区肝癌疾病负担进行长期趋势的分析和比较研究。考虑到HBV疫苗和乙肝抗病毒治疗已经在这些地区开展多年,但是还没有研究将HBV防控措施和肝癌发病率变化联系起来分析。申请人以及所在团队通过对我国和其他东南亚地区7个国家1983-2007年既往报告的肝癌和分型数据进行统计建模,分析和比较肝癌发病率变化的历史趋势特别是HBV防控措施的相关时间节点的关系;同时运用Age-Period-Cohort(APC)模型,首次预测了我国和其他东南亚国家到2030年肝癌发病率变化趋势。本研究的创新性在于首次对东亚和东南亚地区肝癌发病率的长期变化趋势进行研究并通过统计学模型预测该地区直到2030年的肝癌发病率变化情况。该研究的结果不仅可以为既往干预措施对于降低肝癌发病率的有效性提供大数据证据,预测的结果对于未来包括预防、诊断和治疗资源的调配具有重大指导意义。本研究结果发表于胃肠肝病学顶级期刊Gastroenterology(IF=3D19.233),同时该杂志主编Andrew T. Chan和Christopher S.William共同撰写Covering the Cover在当期杂志的首页介绍本研究。

二、首次创建了我国丙型肝炎(HCV)疾病进展和预测数学模型,并运用该模型预测我国至2050年的HCV相关疾病负担情况,同时模拟不同干预场景下的疾病负担变化情况,考虑到世界卫生组织(WHO)动员世界各国在2030年消灭HCV的背景下,本研究首次为我国消灭HCV的行动提供了关键数据和可行性方案,具有重大的科学价值和社会经济价值。申请人以第一作者身份将相关结果发表于胃肠肝病学顶级期刊Gastroenterology(IF=3D19.233)。

 尽管我国普通人群中HCV感染率水平不高,且在世界范围内属于低流行水平,但是我国人口众多,HCV整体疾病负担严重,特别是考虑到HCV能够在较长的时间内导致肝硬化和肝癌等并发症,对我国居民健康危害极大。目前我国针对HCV长期危害方面的研究,特别是基于大数据的预测研究十分缺乏。申请人和其所在团队通过构建个体水平的复杂系统模型,模拟个体感染HCV后疾病进展,首次预测中国HCV及其并发症疾病负担至2050年的变化情况;同时融合不同的干预场景模拟,评估和预测不同干预策略下的疾病负担变化情况。本研究不仅预测了中国未来几十年由HCV所引发的相关疾病负担的变化情况,同时比较了不同策略下对HCV干预效果,为我国制定相应的人群干预和治疗措施提供了重要参考。本研究的创新在于首次设计和构建符合中国HCV流行特点的HCV疾病负担模型,同时该模型设计了非常复杂的疾病转归情况,如:SVR和病人的HCV基因型有关等。并且模拟的结果和国际上非常著名的全球疾病负担研究(Global Disease Burden study) 能够形成交叉验证,具有较高的可信性和可靠性。考虑到世界卫生组织(WHO)动员世界各国在2030年消灭HCV的背景下,本研究首次为我国消灭HCV的行动提供了关键数据和可行性方案,具有重大的科学价值和社会经济价值。

三、构建全球56个国家1960-2012年近50年儿童肺炎死亡率时间趋势模型和Joinpoint回归模型,探索儿童肺炎死亡率变化趋势以及趋势转变的节点信息。本研究对于充分了解全球儿童肺炎死亡率变化趋势及可能的原因意义重大。申请人以共一排一作者身份将相关结果发表于感染病著名期刊Clinical Infectious Diseases (IF=3D9.117)。

 据估计,全球每年有630万5岁以下儿童因为疾病而早死,其中15%是由肺炎引起的。过去十年,儿童肺炎死亡人数大幅减少,从2000年的170万人,减少到2011年的130万人。然而,这些改善在各国之间并不一致。之前的研究并未将不同国家儿童肺炎死亡长期变化趋势进行系统分析,也没有通过统计学模型探索儿童肺炎死亡率变化时间节点和潜在的原因。申请人和其所在团队通过构建时间趋势模型和Joinpoint回归模型,探索全球56个国家1960-2012年近50年儿童肺炎死亡率变化趋势以及趋势转变的节点信息;研究确定了3个具有统计学意义的肺炎死亡率下降期。本研究的主要创新性在于对56个国家的儿童5岁以下肺炎死亡率进行长时间的(超过半个世纪)趋势分析,并创新性地将各国儿童肺炎死亡率的变化趋势和当地的经济发展水平进行关联性分析,同时通过Join-point回归分析发现全球儿童肺炎死亡率变化节点,并将这些节点和既往各国开展的卫生干预结合,为论证既往卫生干预的有效性提供大数据的支持。研究的成果将有助于未来公共卫生政策制定者了解过去50年儿童肺炎死亡率的变化情况,帮助他们在未来的政策制定中认识肺炎死亡率在地区和国家之间的不平衡性,指导未来卫生资源调配。

 


三、岗位工作思路及预&= #26399;目标

1.工作思路:①在未来的工作中,充分利用医院的平台,加大对医院现有数据的挖掘,加大和临床医生的合作,凝练临床实践中的科学问题,通过统计学模型解决科学问题,用数据支持临床实践。②继续深化疾病数学模型研究,构建2-3种疾病的预测模型,为这些疾病的卫生政策制定提供模型支持,同时公开发表这些模型,寻求和其他科研团队的合作机会,扩大研究的国内外影响。③充分利用浙江大学和浙江大学医学院附属第一医院的双一流平台,积极争取和国外研究机构特别是和已经有合作基础的英国Imperial College London和美国UCLA等机构的合作,努力推动研究方向迈向国际先进行列。④ 加强硕士研究生的培养,在专业方向上努力打造青年后备力量,形成稳定的研究团队。

2.预期目标:在未来的5年内,发表SCI论文3-5篇,其中争取1-2篇IF>20。积极申报国家自然科学基金,争取1项面上项目,同时争取国家重大专项或重点研发计划的子课题。
