二、主要学术成就<= /span>

2.1 <= /span>标志性成果(不&#= 36229;过300字)

申请人在生殖健康领域,围绕建立生殖遗传诊治平台和优化助孕技术方面进行科研成果临床转化并取得了以下成果:(一)、参与建立从染色体数目、结构畸变到家系基因突变检测的系统化、多层次的胚胎植入前遗传学诊断技术平台,从而避免传统产前诊断存在的感染风险和异常妊娠后流产带来的身心健康问题,从源头上阻断遗传病的传递。(二)、在国际上首次发现重度少弱精男性精子的非编码RNAmiRNA, piRNA)参与受精卵早期发育并影响体外受精治疗结局。(三)、发现ET-1GDNF等因子以旁分泌形式调节卵子成熟的全新机制,为设计卵子体外成熟培养液打下基础。(四)、在体外受精培养体系和冷冻环节优化多项实验室技术,发明了利用液相芯片进行无创胚胎质量评估的技术。

申请人参加工作以来,一直从事辅助生殖技术和生殖遗传领域的工作,2009年晋升为副研究员,并担任辅助生殖实验室主任,至今已8年。在此期间,主要在建立生殖遗传诊治平台和优化助孕技术方面进行了多项研究并将成果应用到实际工作中。

一:参与建立并完善了从染色体数目、结构畸变到家系基因突变检测的系统化、多层次的胚胎植入前遗传学诊断(PGD)技术平台。

PGD是一种将辅助生殖技术与遗传学诊断技术相结合的新型的产前诊断技术,相比于传统的产前诊断,PGD在胚胎发育的最早阶段即对胚胎进行遗传学分析,把遗传性疾病的控制前移到胚胎植入子宫内膜前,可避免遗传异常妊娠的发生,消除产前诊断异常时选择性流产给孕妇带来的身心创伤,在伦理上也更易被人接受,并作为出生缺陷的早期干预手段而日益受到关注。申请人每年完成近3000例遗传咨询工作,完成200多例PGD胚胎活检工作,针对每个家系进行分析,建立家系特异性的单细胞分子诊断技术,将近年来迅速发展的分子生物学技术,如比较基因组杂交技术,二代测序技术等应用到PGD,诊断范围涵盖了包括染色体数目异常、染色体结构异常、单基因遗传病(进行性肌营养不良,雄激素不敏感综合征,先天性成骨不全等)、性连锁疾病等,出生婴儿诊断符合率100%,成功满了PGD患者出生健康子代的需求。申请人和所在团队在这方面的工作帮助生殖中心在生殖遗传领域处于全国领先水平,目前仍是国内技术最先进、齐全的PGD诊断临床应用平台之一。

任现职以来以第一作者身份发表与PGD相关内容SCI 论文4篇。在国际上首次报道完成了雄激素不敏感综合征的PGD,文章发表在Clinical Genetics上(Ye et al, 2012;82(3):295-296)。发现相互易位携带者的某些特征,如断裂点位置,易位涉及染色体类型以及携带者性别等对PGD获得正常或者平衡易位胚胎的概率有一定影响,为平衡易位患者评估获得正常妊娠的概率,进行相关遗传咨询提供了依据,文章发表在Reproductive Biomedicine Online上(Ye et al.2012 ;24(1):83-90)。

 

二、在国际上首次发现重度少弱精男性精子的miRNA参与受精卵早期发育并影响体外受精治疗结局

卵母细胞和精子结合共同形成胚胎,但传统认为卵源性因子主导了早期胚胎发育直至胚胎基因组激活。申请人在参与国家重大科学研究计划,进行父/母源性生殖障碍诱发胚胎源性疾病风险评估及机制研究(项目号:2012CB944902,申请人为主要科研骨干)的过程中发现重度少弱精症男性精子的miRNA-34c水平与正常男性相比有差异,而且在国际上首次发现miRNA-34c水平与卵浆内单精子注射后受精卵早期发育情况和胚胎着床能力有相关性,提出男性精子可能通过非编码RNA影响受精卵发育和胚胎着床,这可能是部分重度少弱精症患者体外受精结局不佳原因之一。相关研究内容已经发表在Fertility and Sterility上(Cui et al, 2015. 104(2):312-317. 申请人为通讯作者)。除了miRNA, 数个piRNA(piR-31704, piR-39888, and piR-40349)也被发现与精子浓度和卵浆内单精子注射后卵子受精率有相关性,相关内容已经发表在Reproductive Science上(Cui et al, Epub ahead of print)。我们的工作提供了精子参与受精卵早期发育的证据和可能途径,并已申请到了国家自然科学基金(项目编号:81771585,申请人为项目负责人),将深入探讨精子非编码RNA影响受精卵早期发育的机制。

 

三、发现ET-1GDNF因子以旁分泌形式调节卵子成熟的全新机制

申请人在卵母细胞成熟机理方面开展了旁分泌调节促进卵母细胞成熟的研究。既往的研究关注的主要是下丘脑-垂体-卵巢轴对卵母细胞的内分泌调节,申请人研究发现卵巢内部的旁分泌对于卵母细胞的成熟有非常重要的作用。排卵前,在黄体激素(LH)的作用下,引发原来静止的减数分裂重新恢复,启动卵母细胞成熟过程。但哺乳动物的卵母细胞本身不表达LH受体,申请人提出:卵泡中的一些体细胞,如卵丘细胞、颗粒细胞等受到LH刺激后分泌某些生长因子,如内皮素-1,胶质细胞源性神经营养因子(GDNF)等以旁分泌形式作用于卵丘/卵母细胞,促进了卵母细胞的成熟。申请人获得了两项国家自然科学基金的资助(项目编号:8117056781370761,申请人为项目负责人),在研究中发现:内皮素-1GDNF的添加均能在体外培养中提高卵母细胞成熟率。内皮素-1作用于缝隙连接蛋白Cx26调节卵母细胞成熟,文章投稿molecular human reproduction, 已修回。而GDNF能下调卵丘细胞miR-145-5的表达,作用于GFRA1EGFR等靶基因,减少卵丘细胞凋亡,促进卵母细胞核成熟和胞浆成熟。我们的发现既充实了卵母细胞成熟的机理,也有助于在辅助生育治疗中解决卵母细胞体外成熟(IVM)的难题。IVM是指通过体外培养,使卵泡内的未成熟卵母细胞发育成为成熟的卵母细胞,受精分裂成胚胎并移植获得妊娠的技术。IVM可以降低大剂量促排卵药物费用,更重要的是可以避免药物刺激卵巢带来的副作用。但目前IVM技术的成功率仍远低于常规IVF技术,主要原因是对卵母细胞成熟所需要的环境条件尚未完全明确。我们的研究对设计合理的体外成熟培养液具有重大意义,目前已经申请国家专利一项,并获得初审合格(申请号:201510421614.1)。

 

四、在体外受精培养体系和冷冻环节优化多项实验室技术,发明了利用液相芯片进行无创胚胎质量评估的技术

申请人所率领的辅助生殖实验室每年有6000余个体外受精-胚胎移植周期,3000多个冷冻胚胎复苏周期,每年有约3500个家庭通过辅助生殖技术而获得新生儿。申请人进行的助孕新技术和新方法研究获得了国家科技支撑计划的资助(项目编号:N20120266-04,申请人为主要科研骨干)。申请人和所在团队在工作中将国内外最新进展应用到工作中,不断优化助孕技术,包括胚胎培养评估技术、胚胎和配子冷冻复苏技术等是辅助生殖实验室一直保持良好的治疗结局,处于国内先进水平的关键。

体外受精(IVF)的妊娠率与所选择胚胎的发育潜能密切相关,目前通行的方法是根据胚胎的形态进行评分,但是胚胎的形态与实际的发育能力并不完全一致,导致IVF的胚胎着床率在30%左右,即一个胚胎移植到子宫以后,只有约30%机会继续发育为胎儿,而且,形态学观察具有一定主观性和个体差异。申请人利用近年来出现的液态芯片技术,开发了胚胎发育潜能预测液态芯片,胚胎培养液和卵泡液中的多个成分进行快速、无创的定量分析,作为对胚胎形态学分析方法的有益补充。这方面的研究获得了浙江省科技厅重大专项的资助(项目编号:2011C11033,申请人为项目负责人)。经过研究发现,使用潜能预测液态芯片筛选后的胚胎发育潜能优于一般胚胎,着床率明显增加,改善了体外受精-胚胎移植治疗结局。该项成果已经获得国家发明专利(专利号:201310710571.X申请人为第一发明人)。国内已有数百家生殖中心,如果能够开发胚胎发育潜能预测的液态芯片诊断试剂盒,将会有着可观的市场前景。且国内尚未有同类试剂,该产品的开发将会弥补相关产品类的空白。

在胚胎的形态学观察方面,我们也进行了改进,增加了受精卵观察时期的一项特殊指标,胞浆质晕的观察,发现受精卵胞浆质晕的出现预示较好的胚胎发育潜能,增加该项指标后临床妊娠率从42.8%增加到50.6%。相关研究内容发表于生殖与避孕杂志(2013339):581-585。基于上述研究等改进胚胎评估选择技术的基础上,生殖中心的妊娠率从2013年前的40%左右提高至目前的45%以上。

冷冻技术是辅助生殖实验室不可或缺的重要操作。一次体外受精治疗可获得多个胚胎,将部分或全部胚胎冻存,在适当的时候复苏移植到患者子宫,可以提高累积妊娠率,也为不适合新鲜周期移植的患者选择胚胎移植时机提供了可能。我们和试剂生产厂家合作进行关于胚胎程序化冷冻的研究发现:提高细胞外冷冻保护剂浓度可将冷冻复苏周期妊娠率从43%提高至52%,相关内容发表于Journal of Assisted Reproduction and Genetics上(Fang et al, 2016, 33(10), 1389-1393, 申请人为通讯作者)。精子冻存是一项较为成熟的技术,但冻存极重度少弱精男性精子或睾丸活检获得的极少量精子则是一项挑战,采用常规方法冻存后这些精子失活或者丢失的风险较高,我们采用冻存胚胎的玻璃化冷冻技术,将少量精子以微量体积冻存与特殊的载体上,获得了满意的效果。相关研究内容发表于Andrologia上(Chen et al, 2015, 47(6):694-699, 申请人为通讯作者)

申请人在体外受精实验室胚胎培养和评估、配子和胚胎冷冻复苏等多个环节进行的基础科研成果临床转化改善了辅助生殖治疗的结局,相关研究获得了省科技进步二等奖(2016,排名第三)和全国妇幼健康科学技术奖一等奖(2015,排名第五),更降低了不孕不育夫妇费用负担,促进了家庭稳定和社会和谐,具有良好的社会效益。




三、岗位工作思路&= #21450;预期目标

继续从事辅助生殖技术和生殖遗传临床工作,及时将国内外和生殖实验室团队研究成果进行规范系统的临床转化,提升不孕症治疗效果,加强生殖遗传疾病早期防控。凝练研究方向,揭示与临床干预相关的配子发生和胚胎早期发育的相关机制。

1.       在配子发生和早期胚胎发育机制研究方面,在前期研究发现男性精子通过非编码RNA影响受精和胚胎发育的基础上,深入探讨精源性非编码RNA参与胚胎早期发育的机制,争取发表高影响因子的论文。在前期发现卵巢旁分泌因子调节卵母细胞成熟的基础上,继续卵母细胞成熟机制研究,探索更有效的体外成熟培养液,改善辅助生殖治疗结局。

2.       在胚胎植入前遗传学诊断(PGD)方面,将新的适用的分子生物学技术应用到PGD,探索检测培养液游离DNA的无创PGD技术,建立更安全可靠、应用范围更广的PGD技术平台,有效避免遗传异常妊娠的发生。不断加强我院在PGD项目上的优势,通过临床研究、病例报道等方式发表论文,扩大国内外学术影响力。

3.       作为辅助生殖实验室负责人,制定学科方向,组织实验室人员更高效地开展辅助生殖临床实验和科学研究,优化辅助生殖实验室技术,及时将最新成果进行临床转化,不断提高为不孕患者服务的水平。同时,将我们的经验撰写论文,为其它辅助生殖专业人士提供参考。

4.       参与医学院教学工作,培养研究生,以卫生部辅助生育实验室技术培训基地为依托,培训全国辅助生殖实验室技术人员,将相关技术对外辐射。
