二、主要学术成就<= /span>

2.1 <= /span>标志性成果(不&#= 36229;过300字)

(1)发现H7N9禽流感感染来源及重要致病机制。证明活禽市场是人类感染H7N9主要源头,发现患者有“细胞因子风暴”现象,患者抗体水平与预后密切相关。研究结论为H7N9禽流感防治措施制定提供科学依据。(2)创建了乙肝大数据时空预测和乙肝疫苗人群免疫屏障模型。首次发现20岁以下人群乙肝HBsAg携带率1.51%,证明乙肝疫苗接种覆盖率需达到64%才能建立人群保护性免疫屏障,为传染病重大专项“十三五”战略报告制定提供依据。(3)系统阐明了SARS后中国45种传染病的流行病学特征和演变。发现我国手足口病,乙肝和结核病发病率最高,包虫病,丙肝和梅毒发病增长最快;在儿童和男性传染病总发病率较高;我国总的传染病发病率逐渐上升,但在2009年后呈现平稳趋势。

1. 本人作为主要研究者,发现H7N9禽流感感染来源及重要致病机制。证明活禽市场是人类感染H7N9主要源头,发现患者有“细胞因子风暴”现象,患者抗体水平与预后密切相关。研究结论为H7N9禽流感防治措施制定提供科学依据。研究成果在国际顶级医学期刊LANCET、Journal of Infectious Diseases等杂志发表,获得国际同行高度评价。

(1)在国际上首次证明H7N9病毒源于禽类,再向人类传播。研究发现所有4患者都具有接触家禽史,包括销售或者采购活禽,且研究人员对与患者可能相关的活禽市场中鸡、鸭、鹌鹑和鸽子进行了采样,并检测出H7N9病毒。根据患者体内分离出的H7N9病毒与活禽市场的鸡中分离出的H7N9病毒株进行遗传学比较,发现病毒株之间的基因序列高度同源,首次获得了H7N9病毒从禽类向人类传播的证据。研究还发现H7N9禽流感不同于其他类型的禽流感,在人类感染该病毒之前并没有导致家禽发病与死亡。为我国关闭活禽市场提供了科学的理论依据,通过对活禽市场的积极干预,有效控制传染源和切断传播途径,对H7N9禽流感防控收到了显著成效,短时间内遏制了新发病例的出现。

(2)在国际上首次警示病毒正在向适合感染哺乳动物方向发展,必须密切关注与预防。研究团队对未经防护的303名患者家属和密切接触者,特别是82名与患者有过接触的医护人员进行临床监测,在14天医学观察期内,所有接触者没有出现任何感染征象,提示H7N9病毒目前尚未建立有效的人际间传播方式。但是,研究团队对病毒基因组研究显示,H7基因的226氨基酸位点发生变异,使得更容易感染人。PB2基因尽管只发现了701位点发生变异,但要高度警惕,如果PB2基因627和701氨基酸位点同时发生变异,传播能力将增强。对科学预测H7N9禽流感病原变异变迁流行规律做出了重要的贡献。

(3)首次发现重症患者存在类似SARS的“细胞因子风暴”,而轻症患者无此现象。李兰娟院士在疫情发生初期,即敏锐地提出了重症患者的发病机制与细胞因子介导的严重炎症反应有关。通过对多名重症病例监测证实了在疾病急性期均存在多种细胞因子显著升高的现象,出现了类似当年SARS患者中存在的“细胞因子风暴”现象,解释了H7N9禽流感大多为重症感染的原因。

(4)首次证实H7N9病毒在下呼吸道检测阳性率高,病毒阳性持续时间长。而上呼吸道阳性率低,持续时间短。这个发现对于病原检测、抗病毒治疗具有重要意义。

《柳叶刀》杂志还专门邀请了世界著名病毒学家荷兰阿姆斯特丹大学的Marion Koopmans教授作了亮点评述,给予了高度评价,认为该研究成果为全球H7N9禽流感防治提供了指南。

2. 创建了乙肝大数据时空预测和乙肝疫苗人群免疫屏障模型。首次发现20岁以下人群乙肝HBsAg携带率1.51%,证明乙肝疫苗接种覆盖率需达到64%才能建立人群保护性免疫屏障,为传染病重大专项“十三五”战略报告制定提供依据。研究成果发表在Scientific Reports、Medicine等杂志。

本人在浙江省乙肝防治示范区研究中,对317961名居民进行两次或两次以上大规模乙肝(HBsAg和HBsAb)筛查和随访,以发现新发感染率及其影响因素,建立乙肝免疫屏障效果模型。研究发现5401人为新发感染,按性别和年龄调整后,新发感染率为0.81/100人年。具有HIV/AIDS感染史、在出生前1992人群、沿海居民、具有乙肝家族史以及流动人口等人群新发感染率显著增高,而乙肝疫苗接种和参加体育锻炼等人脉群新发感染率显著较低。乙型肝炎表面抗体阳性率与新发感染率呈负相关(r=3D-0.826)。线性拟合得到新发感染率为1.23+0.02ΧHBsAb阳性率。通过研究首次发现了我国浙江省成人乙肝新发感染率高达0.81/100人年,提出了人群中乙肝疫苗接种覆盖率需达到约64%才能建立人群保护性免疫屏障、有效控制乙肝流行,建议在我国对1992年(新生儿计划免疫实施)之前出生的人群推行乙肝疫苗接种策略

身体穿刺是一种逐渐流行的社会文化现象,但也可能是乙型肝炎病毒和丙型肝炎病毒的一个潜在的暴露途径。目前,已有较多的有关身体穿刺是否增加乙肝病毒和丙型肝炎病毒传播的风险研究,但结论仍然还存在争论。本研究对2015年1月15之前,MEDLINE、EMBASE、万方、CNKI等数据库发表相关文章,谷歌和谷歌学术等数据库的相关灰色文献等进行了详尽的相关搜索研究。共有40项研究被纳入该系统评价,身体穿刺与HBV、HCV传播之间关联的分别为1.80和1.83。亚组分析表明足球和退伍军人中身体穿刺感染丙型肝炎病毒风险最高4.63,而学生/社区群体中身体穿刺感染乙肝风险最高2.40。此系统回顾和荟萃分析表明,身体穿刺与乙肝病毒和丙肝病毒传播显著相关,身体穿刺可能会增加其感染的风险。从这项研究中的证据强烈建议,应制定全面和有效的规范,以提高身体穿刺实践的安全性。

本人进一步通过系统评价和荟萃分析,对影响成人乙型肝炎疫苗反应的因素做出更准确的评估。我们纳入的研究包括对健康成年人群按照免疫接种程序完成乙肝疫苗接种后并检测其血清保护率研究。这项Meta分析纳入了37篇文章21053名成人,结果显示成人(≥40岁)(RR:1.86),男性成年人(RR:1.40),BMI≥25的成人(RR:1.56),吸烟者(RR:1.53)和伴发疾病的成年人(RR:1.39)。同时,我们进一步发现,成人(年龄≥30)(RR:1.77)和成人(≥60岁)(RR:1.30)。然而,在乙肝疫苗在饮酒人群中对乙型肝炎疫苗的应答没有差异(RR:0.90);同时在0-1-12与0-1-6接种程序组中对乙型肝炎疫苗的应答也没有差异(RR:1.39)。这些研究的汇集表明,成人乙肝疫苗策略实行越早越好。应在老年人、男性、BMI≥25、吸烟和伴发基础疾病的易感人群中应增加疫苗剂量或补充强化免疫。常规的0-1-6疫苗接种计划仍值得推荐。

3. 系统阐明了SARS后中国45种传染病的流行病学特征和演变。发现我国手足口病,乙肝和结核病发病率最高,包虫病,丙肝和梅毒发病增长最快;在儿童和男性传染病总发病率较高;我国总的传染病发病率逐渐上升,但在2009年后呈现平稳趋势。研究成果发表在LANCET Infectious Diseases等杂志。

在非典后时代,中国的传染病防控的模式发生了显著的变化,但目前尚没有关于传染病的发展趋势和流行病学特征的研究。方法:利用国家公共卫生科学数据中心及国家卫生计生委网站公开的法定传染病数据,分析2004年至2013年期间我国法定传染病的发病率和病死率。运用jointpoint回归模型来估计发病率的年度变化百分比(APCs)变化趋势。结果:在研究期间,共有45种法定传染病及其亚型的54984661病例被报告(年平均发病率=3D417.98/10万)。其中发病率最高的有手足口病(114.48/10万),乙肝(81.57/10 万)及结核病(80.33/10万)。54984661例病例中有132681例死亡(平均死亡率=3D1.01/10万;平均病死率=3D2.4/1000)。男性总体发病率高于女性,年龄10岁以下人群发病率最高。在20岁以上人群中,男性总病死率高于女性,80岁以上人群总病死率最高。研究期间年平均发病率从2004年的300.54/10万升高到2013年的483.63/10万(APC=3D5.9%),其中,包虫病、丙肝,梅毒增长最快,总体上升趋势在2009年之后发生了改变,2009-2013年的APC(2.3%)显著低于2004-2008年的6.2%。结论:尽管传染病的总发病率是逐渐增长的,但是在2009年以后趋于平稳。对于发病率最高的传染病,包括手足口病、乙肝和结核病,以及发病率上升最快的传染病,包括包虫病、丙肝和梅毒,应采取更为有效的防控措施。澳大利亚皇家墨尔本医院吉布尼(Katherine B Gibney)等在同期述评中写道:传染病的流行病学及其相对疾病负担,决定了哪些传染病应得到优先公共卫生资源和干预。在中国这样的超级大国中,确定引发最大疾病负担的传染病以及受特定传染病影响最大的人群,是一项极为重要的任务。中国国家监测数据10年纵向分析,代表了现代中国国家法定传染病流行病学的综合图卷。公共卫生决策制定者应采用这些结果,从而为公共卫生行动确定疾病和人群优先顺序。

 


三、岗位工作思路&= #21450;预期目标

1. 岗位工作思路:

(1)在流感、禽流感等新发传染病防治研究方面:

紧密结合基础与临床,应用各种现代研究生物技术,深入、全面、系统地研究流感、禽流感等新发传染病流行病学演变规律,重点跟踪H7N9禽流感病毒基因序列的变化;进一步开展流感、禽流感的时空传播模型及预警预测应用研究。运用大数据技术对物理空间和网络空间数据进行多维统合;研究构建流感、禽流感时空演变和传播的预警预测模型,为爆发流行防控提供科学依据;研究耦合于智能体模型的疾病爆发早期预警模型,并利用模型模拟结果制定决策优化干预方案。

(2)在乙肝等重大传染病防治研究方面:

乙型肝炎和由其导致的肝癌缺乏理想的防治方法,急需新的研究突破,通过从临床个体病例追踪拓展乙肝等重大传染病研究队列,建立和评价乙肝疫苗免疫屏障的模式,分析不同乙肝疫苗免疫屏障水平下的HBsAg携带率和HBsAg新发率差异及其相关影响因素,同时分析评价不同队列各种干预措施的干预效果。开展乙肝等重大传染病防治干预措施的成本和费用及其疾病负担研究,建立社区防治经济效益评估系统,为乙肝等重大传染病防控措施的制定提供依据。

(3)在大数据分析挖掘和医疗人工智能研究方面:

以“互联网+”为驱动,运用主数据管理解决技术(MDM)对示范区研究数据进行多维整合,维护并升级示范区信息云平台,提升信息平台运行的稳定性、响应的及时性和数据的安全性。对大数据进行深度挖掘,设计开发自助式可视化分析(SVA)平台,开发面向管理者、医务工作者和普通居民的APP等智能应用服务,探索“数据驱动”的模式创新研究方法,提高基于数据服务的转化研究能力,更好地适应示范区大规模数据和队列研究需求。

2. 预期目标:

(1)在流感、禽流感等新发传染病防治研究、乙肝等重大传染病防治研究以及大数据分析挖掘和医疗人工智能研究方面,形成稳定的研究方向;

(2)在未来5年内发表5-10篇本领域高水平学术论文;

(3)形成一支交叉学科研究团队,培养具有独立研究能力的青年骨干和研究生5-10名;

(4)创建传染病大数据云平台及传染病流行病学和控制数学模型研究中心:在十二五国家传染病信息云平台基础上继续收集各类传染病大数据,并建立各类数学模型的技术方法,研究流感、禽流感等新发传染病、甲肝、乙肝等重大传染病流行病学和控制为核心各类数学模型,使该领域在国内具有领先地位。
